Diyabetes Mellitus’lu Periferik Nöropatili Hastalarda Postüral Denge ve Koordinasyon Problemlerinin Tele-Değerlendirme Sonuçları: İki Olgu Sunumu

Diabetes mellitus’un yaygın komplikasyonlarından biri de diyabetik nöropatidir. Diyabetik nöropati, artmış morbidite ve maluliyetin en sık nedenlerinden biridir ve belirli sıklıklarla klinik değerlendirmeler gerektirir. Tele-değerlendirme hastanın servise gelmeden hem zaman tasarrufu yapmasını hem de kendini evinde olduğu için daha güvende hissetmesini sağlar. Çalışmanın amacı diabetes mellitus’lu periferik nöropatili hastalarda postüral denge ve koordinasyon problemlerinin tele-değerlendirme ve klinikte yüz yüze yöntemlerle elde edilen değerlendirme bulgularını karşılaştırmaktır. Çalışmamıza 2 olgu dahil edilmiştir. Dahil edilen olgulara Romberg, süreli kalk-yürü testi, 5 Defa oturup kalkma testi, koordinasyon testlerinden basamak testi, topuk-parmak ucu yürüme testi, ayakla tempo tutma testi ve topuk tibia testleri uygulanmıştır. Süreli kalk ve yürü testi süresinin ilk olgu için oldukça benzer olduğu, ikinci olgumuzda ise tele ve yüz yüze değerlendirme arasında belirgin farklılık olduğu görülmektedir. yüz yüze yöntem ile değerlendirmede sürenin uzadığı görülmüştür. Koordinasyon testlerinden topuk parmak ucu yürüme testi hariç diğer testlerde tutarlılık olduğu görülmüştür. Bu vaka raporu ile periferik nöropatili hastalarda gelişen postüral denge ve koordinasyon problemlerinin tele-değerlendirme yöntemi ile etkin bir şekilde gerçekleştirilebileceği görülmüştür.

One of the common complications of diabetes mellitus is diabetic neuropathy. It is one of the most common causes of increased morbidity and disability and requires clinical evaluations at certain frequency. With tele-assesment, the patient both saves time and feels safer because patient is at home before coming to the service. The aim of the study is to compare the results of postural balance and coordination problems in patients with peripheral neuropathy with diabetes mellitus, obtained by tele-assesment and clinical face-to-face methods. 2 cases were included in our study. Romberg, timed get up and go test, 5 times sit to stand test, and from coordination tests; step test, heel toe walking test, foot tempo holding test, heel-tibia test were applied to the patients. It was seen that the duration of the timed get up and go test is quite similar for the first case, and there was a significant difference between the tele-assesment and face-to face assesment in our second case. It was observed that the assesment time increased in face-to-face method. Coordination tests were consistent with the exception of the heel toe walking test. With this case report, it was observed that postural balance and coordination problems that emerging in patients with peripheral neuropathy can be effectively realized with the tele-assessment method.
