Preeklampsinin emzirme özyeterliliği ve postpartum depresyon üzerine etkisi

Özet:Giriş- Amaç: Bu çalışmanın amacı preeklampsinin emzirme öz-yeterliliği ve postpartum depresyon üzerine etkisini ve etkileyen faktörleri belirlemektir. Yöntem: Tanımlayıcı ve kesitsel tipteki bu araştırma, Ocak 2018- Mayıs 2019 tarihleri arasında İzmir Sağlık Bilimleri Üniversitesi Tepecik Eğitim ve Araştırma Hastanesi'nde sezaryen ile doğum yapmış örneklem kriterlerine uyan 73 preeklampsili anne ile yapılmıştır. Veriler, Birey Tanıtım Formu, Edinburgh Postpartum Depresyon Ölçeği (EPDÖ) ve Emzirme Öz-Yeterlilik Ölçeği (EÖYÖ) kullanılarak ve iki izlem yapılarak toplanmıştır. Birinci izlem doğum sonrası 24 saatini doldurmuş annele . . .re yüzyüze görüşülerek hasta odasında, ikinci izlem doğumdan sonra ikinci ayda telefon ile görüşülerek yapılmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistikler (sayı, yüzde, ortalama), Mann Whitney U, Kruskal Wallis testi, Wilcoxon testi ve korelasyon analizi kullanılmıştır. Bulgular: Doğum sonrası 24. saatte preeklampsili annelerin depresyon riski 16,4 iken, ikinci ayda bu oran 9,6'dur. Çalışmada gebelik sayısı dört ve üzeri, gebeliği plansız, ikinci gün emzirmeye başlayan ve emzirme için desteği olmayan annelerin 24. saat depresyon riski daha yüksek iken emzirmeye ilişkin gebelik sürecinde eğitim alan annelerin ikinci ay depresyon riski daha düşüktür. Gebeliği planlı olan emzirmeye ilişkin bilgi alan ve desteği olan, anne sütü dışında başka gıda vermeyen annelerin hem 24. saat hem de ikinci ay emzirme öz yeterlilikleri daha yüksektir. Annelerin doğum sonrası 24. saat EÖYÖ puan ortalaması ile ikinci ay EÖYÖ puan ortalamaları arasında istatistiksel olarak anlamlı, pozitif yönde güçlü bir ilişki vardır. Sonuç: Araştırmadan elde edilen bulgulara göre preeklampsili annelerin emzirme öz-yeterliliği düşüktür ve gebeliğin planlı olması, emzirmeye ilişkin bilgi alma durumu, destek olabilecek kişiye sahip olması ve anne sütü dışında başka gıda verme durumundan etkilenmektedir. Preeklampsili annelerin postpartum depresyon riski ikinci ayda, 24.saate göre azalmış olmasına rağmen halen risk devam etmektedir. Anahtar Kelimeler: Preeklampsi, gebelik, emzirme öz-yeterliliği, postpartum depresyon, hemşirelik.Summary:Objective: The aim of the study was to determine the effect of preeclampsia on breast feeding self-efficacy and post partum depression. Method: Descriptive and cross-sectional study was conducted between January 2018- May 2019 in Tepecik Research and Training Hospital of Izmir Health Sciences University. 73 mothers that comply with acceptance criteria, gave birth with caesarean surgery and has preeclampsia were accepted in the study. Data were collected with Personal Identity Form, Edinburg Postpartum Depression Scale (EPDS) and Breastfeeding Self Efficacy Scale (BSES) in two monitoring sessions. The first follow-up was conducted in patient room with face-to-face interview together with mothers that completed their 24 hours after the birth. The second follow-up was conducted by telephone conversation in the 2nd month after the birth. Descriptive statistics (number, percentage, average), Mann Whitney U, Kruskal Wallis Test, Wilcoxon Test and Correlation analysis were used in data evaluation. Results: The depression risk of mothers that had preeclampsia was 16,4 in the 24th hour of birth, while it was 9,6 in the second month. In this study it was found that depression risk of mothers in their 24th hour was higher for mothers that gave birth four or more, had unplanned pregnancy, started breastfeeding in the second day and did not have any support for breastfeeding. It was found that second month depression risk was less for mothers that had training in their pregnancy. Self efficacy were found higher for both 24th hour and 2nd month of mothers that had a planned pregnancy, had a support and training about breastfeeding and didn't give anything else but breast milk. In the study, a positive strong relationship was found between BSES score average of 24th hour and 2nd month of mothers after the birth. Conclusion: According to results found in the study, self efficacy of mothers that had preeclampsia was low and it was effected by the planned pregnancy, getting training about breastfeeding, having a supporting person and not giving anything else than breast milk. However, postpartum depression risk of mothers that had preeclampsia were lower in their second month, risk was still in progress. Özet:Bu araştırma, gestasyonel diyabetin gebelik ve doğum sonrası dönemde depresyon ve emzirme öz-yeterliliği üzerine etkisini incelemek amacıyla tanımlayıcı ve karşılaştırmalı olarak yapılmıştır. Araştırma, Temmuz 2016-Haziran 2017 tarihleri arasında iki üniversite hastanesine rutin prenatal izlem için gelen gebelerle iki aşamalı olarak yürütülmüştür. Birinci aşamada (34-38. haftalar arası) 104 gestasyonel diyabetli (GDM), 133 GDM'li olmayan gebe ile görüşülmüş ikinci aşamada (postpartum 8. haftada) ise ilk aşamada görüşülen ve telefonla ulaşılabilen 74 GDM'li, 103 GDM'li olmayan anne ile çalışma tamamlanmıştır.
