COVID-19 Pandemi Sürecinde Dünyada Hemşirelik Eğitimi

Bu derlemede, COVID-19 pandemisi sırasında farklı ülkelerdeki hemşirelik okullarında, ülkelerindeki mevcut koşullar, yasal düzenlemeler ve mesleki kurumların önerileri doğrultusunda yapılan uygulamaları açıklamak ve örnekleri ile sunmak amaçlanmıştır. COVID-19 pandemisinin ortaya çıkışı, hemşirelik okullarındaki eğitim şeklinde hızlı değişiklikler yapılmasını, yenilikçiliği, esnekliği ve hızlı hareket etmeyi gerektirmiştir. Hemşirelik eğitiminde yüz yüze yürütülen teorik dersler hızla çevrim içi ortamlara taşınmıştır. Dünyanın birçok bölgesinde virüsün doğası ve hakkındaki belirsizlik, kişisel koruyucu ekipman temini, sağlık sigorta . . .sı, eğitimci denetimi gibi nedenler göz önüne alınarak öğrencilerin klinik uygulamaları iptal edilmiştir. Bu nedenle klinik uygulama ile öğrencinin kazanabileceği bilgi, beceri ve tutumların geliştirilmesi tüm dünyada hemşirelik eğitiminde en çok zorluk yaşanan alan olmuştur. Çevrim içi eğitime geçiş, okulların öğrenci katılımını teşvik edecek stratejiler geliştirmelerini, sınav prosedürlerinde ve not verme uygulamalarında değişiklikler yapmalarını gerektirmiştir. Aynı zamanda eğitimciler öğrencilere alternatif klinik deneyimler sunmak ve öğrenci performansının nasıl değerlendirileceğini yeniden tanımlamak zorunda kalmışlardır. Hemşirelik okulu yöneticileri ve eğitimcileri pandemi süresince hemşirelik eğitiminin devamlılığını sağlamak ve kalitesini sürdürmek için okullarda altyapının düzenlenmesi, hemşirelik öğrencileri için klinik alan gereksinimlerinin planlanması, akreditasyon standartlarının sürdürülebilirliğinin sağlanması gibi pek çok unsuru göz önünde bulundurmuştur. Salgınlarla mücadelede yetkin sağlık profesyonelleri yetiştirebilmek için eğitimde gerekli düzenlemeler acil olarak yapılmalıdır. The aim of this review is to explain and present examples of the practices in nursing schools around the world during the COVID-19 pandemic, considering the conditions, legal regulations, and recommendations of professional associations in different countries. The emergence of the COVID-19 pandemic has required rapid changes in the form of education in nursing schools as well as innovation, flexibility, and rapid action. Theoretical courses conducted face-to-face in nursing education have been rapidly transferred to online platforms. In many parts of the world, the clinical learning has been canceled, considering the uncertainty of the virus, the supply of the personal protective equipment, health insurance, and educator supervision. For this reason, developing the knowledge, skills, and attitudes that students gain through clinical learning has been the most difficult issue in nursing education all over the world. The transition to online education has required schools to develop strategies to encourage student participation, and to make changes in exam procedures and grading. Meanwhile, educators had to offer students alternative clinical experiences and redefine how to evaluate student performance. To ensure the continuity of nursing education and to maintain its quality during the pandemic, nursing school deans and nurse educators have taken into account many factors such as organizing the infrastructure in schools, planning clinical learning requirements for nursing students, and ensuring the sustainability of accreditation standards. Dünya Sağlık Örgütü tarafından COVID-19 olarak adlandırılan yeni koronavirüs, kısa bir sürede tüm dünyayı etkisi altına almıştır. Hızla yayılan bulaşıcı bir hastalık olup temas ve solunum damlacıkları ile bulaşmaktadır. COVID-19 açısından pek çok riskli grup vardır. Gebeler ve yenidoğanlar bu gruplar içerisinde önemli bir yer tutmaktadır. COVID-19 tanılı annelerin antenatal, intrapartum ve postpartum süreçleri de anne-bebek sağlığı açısından bireylerde endişe yaratabilmektedir. COVID-19’un transplasental olarak anneden bebeğe geçip geçmediğine dair mevcut kaynaklar az olmakla birlikte, emzirme sırasında ve gerekli önlemler alınmadığ . . .ında solunum damlacıkları ile geçebileceği düşünülmektedir. Ayrıca bu süreçte yenidoğanın beslenmesi ve emzirme süreci de tartışılır bir konu haline gelmiştir. Pek çok sağlık kuruluşunun önerileri incelendiğinde bu süreçte anne sütü ile beslenme önerilmektedir. Pandemi sürecinde anne bebek sağlığının sıkça ele alınması, hemşireler tarafından verilecek eğitim ve danışmanlıklar ile enfekte annelere doğum süreçlerinde rehberlik edilmesi, gerekli noktalarda danışmanlık ve destek sağlanması, nitelikli bakımlar ile yenidoğan sağlığının korunması, sürdürülmesi ve geliştirilmesi pandeminin anne-bebek sağlığı üzerine olumsuz etkilerinin en aza indirilmesine ve olası komplikasyonların önlenmesine yardımcı olacaktır. Bu derlemenin amacı, COVID-19 pandemi sürecinde anne-bebek sağlığı, yenidoğan beslenmesi ve emzirme süreçlerine dair bilgilerin sunulmasıdır. The new coronavirus, named COVID-19 by the World Health Organization, has taken the whole world under its influence in a short time. It is a rapidly spreading infectious disease and is transmitted by contact and respiratory droplets. There are many risky groups regarding COVID-19. Pregnant women and newborns have an important place among these groups. Antenatal, intrapartum, and postpartum processes of mothers diagnosed with COVID-19 may also cause anxiety in individuals in terms of mother-infant health. Although there are few resources available on whether COVID-19 is transmitted from mother to baby transplacentally, it is thought that it can be transmitted via respiratory droplets during breastfeeding and when the necessary precautions are not taken. In addition, the feeding and breastfeeding process of the newborn has become a debated issue in this process. When the recommendations from many health institutions are examined, breastfeeding is recommended in this process. Addressing the maternal and infant health frequently during the pandemic process, guiding infected mothers in their birth processes via training and counseling by nurses, providing consultancy and support where necessary, protecting, maintaining, and improving newborn health via quality care will help minimizing the negative effects of the pandemic on maternal and infant health and preventing possible complications. COVID-19 Pandemi Sürecinde Uzaktan Hemşirelik Eğitimi: Ulusal Deneyim

COVİD-19’un Dünya Sağlık Örgütü tarafından bir pandemi olarak tanımlanmasını takiben üniversitelerde yüz yüze olan eğitime ara verilmiş ve uzaktan eğitim sürecine geçilmiştir. 2020-2021 eğitim öğretim yılı güz dönemi uzaktan eğitim şeklinde, üniversitelerin olanakları ölçüsünde çevrimiçi/senkron, asenkron ya da hibrit şekilde yürütülmüştür. Bu süreçte hemşirelik lisans eğitim programımız “Hibrit Eğitim Modeli”ni kullanmıştır. Uzaktan eğitim sürecimizde müfredatta yer alan derslerde herhangi bir sınırlandırma, azaltma, başka bir döneme erteleme yapılmadan tamamı aksatılmadan yürütülmüştür. Derslerin teorik bölümleri çevrimiçi/senkron . . . veya asenkron olarak işlenmiştir. Laboratuvar uygulamaları ve klinik/saha uygulamaları ise COVİD-19 salgınında alınması gereken önlemler alındıktan sonra yüz yüze yürütülmüştür. Ebelik ve hemşirelik meslekleri toplumlarda bakımın ilk ve tek noktası olduklarından dolayı Dünya Sağlık Örgütü 2020 yılını Uluslararası Ebe ve Hemşire yılı olarak belirlemiştir. Ebeler, gebelik, doğum ve postpartum gibi kritik dönemlerde, kadın ve yenidoğanların sağlığının korunmasında merkezde yer almaktadır. Ebeler ayrıca kadınlar için doğum kontrol yöntemlerine ve diğer üreme sağlığı hizmetlerine erişiminin sağlanmasında önemli bir rol oynamaktadır. Dünya Sağlık Örgütü, tüm anne ölümlerinin, ölü doğumların ve yeni doğan ölümlerinin %83’ünün ebelik bakım programları ile önlenebileceğini tahmin etmektedir. As midwifery and nursing professions are the first and only point of care for the populations, the World Health Organization has determined the year 2020 as the International Year of Midwives and Nurses. During critical periods such as pregnancy, delivery and postpartum, midwives are in the center to protect the health of women and newborns. Midwifes also play an important role in providing access to contraception and other reproductive health services for women. The World Health Organization estimates that 83% of all maternal deaths, stillbirths and newborn deaths can be prevented by midwifery care programs. However, in the battle of COVID-19 management, maternal and neonatal healthcare have become less of a priority in some countries. COVID-19 günlük hayatın akışını etkilediği için küresel bir kriz olarak tanımlanmaktadır. COVID- 19 hasta bakımı ve izleminde etkin görev alan hemşireler kriz sürecinde hasta bakımında en iyi sonuca ulaşmayı hedeflemektedir. Kuramlar mesleğinin gelişmesine katkıda bulunmakta hemşirelere doğru olanı uygulama konusunda rehber olmaktadır. Fitzpatrick’in ritim kurama göre; kriz anında bireyin bütünlüğü tehdit altında kalmaktadır. COVID-19 is defined as a global crisis because it affects the flow of the daily life. Nurses who play an active role in the patient care and follow-up processes of COVID-19 aim to achieve the best results in patient care during the crisis period. Theories contribute to the development of the nursing profession and they guide nurses regarding providing the right care. According to Fitzpatrick's rhythm theory, in case of crisis, the integrity of the individual is under threat. The crises experienced by the individual specify the rhythmic exits in the development process of the individual, and affect his/her temporal, motional, cognitional, and perceptual behaviors. In this qualitatively planned study, the experiences of a charge nurse of a COVID-19 pandemic department were evaluated according to the Fitzpatrick’s rhythm theory. COVID-19 Pandemisinde Okul Sağlığı Hemşirelerinin Rol ve Sorumlulukları

Özet COVID-19 enfeksiyonu, dünyayı kısa süre içerisinde etkisi altına almış ve 2020 yılının başında Dünya Sağlık Örgütü tarafından pandemi ilan edilmiştir. Salgının kontrol altına alınması amacıyla hükümetler tarafından sokağa çıkma ve seyahat kısıtlamaları, topluluk halinde yapılan etkinliklerin iptal edilmesi, evden çalışma, uzaktan eğitim ve hastanelerde acil dışındaki diğer sağlık hizmetlerinin sınırlandırılması gibi toplumun her kesimini etkileyen kararlar uygulanmıştır. Çocukların ve ailelerin yaşamını etkileyen önlemlerden bir tanesi, okulların kapatılmasıdır. Okulların kapanması ile çocukların yüz yüze eğitimi kesintiye uğr . . .amış, teknolojik alt yapısı yetersiz olan okullarda okuyan ve yeterli teknolojik ekipmanı olmayan aileler, eğitim hizmetine erişimde güçlük yaşamışlardır. Ayrıca çocuk ve ailelere verilen sağlıklı beslenme, aşılama, sağlık izlem ve kontrollerinin sürdürülmesi gibi okul sağlığı hizmetlerine erişimde aksaklıklar meydana gelmiştir. Bu süreçte uzun süreli ev karantinaları ile başta şiddet vakaları olmak üzere, aile içi sorunlarda artış olduğu bildirilmiştir. Pandeminin seyrine göre bazı ülkelerde okullar, kısmi veya tam zamanlı olarak yeniden açılmıştır. Ülkemizde de okulların yeniden açılması konusunda belirli düzenlemeler yapılmış ve pandemi kontrolüne yönelik bireysel ve kurumsal bir dizi önlem ve uygulama gündeme gelmiştir. Ülkemizde özellikle devlet okullarında görevli okul sağlığı hemşiresi bulunmadığından çoğu okulda pandemi kontrolünü öğretmenler üstlenmişlerdir. Pandemi döneminde tüm dünyada önemi artan hemşirelik hizmetleri okul toplumu için de vazgeçilmezdir. Profesyonel bir sağlık çalışanı olan okul sağlığı hemşireleri, ülke nüfusunun önemli bir bölümünü oluşturan okul toplumunun sağlığının korunması ve sürdürülmesinde anahtar role sahiptir. Ülkemizde her okulda okul sağlığı hemşiresi bulunmasa da, pandemi yönetiminde eğitim ve sağlık arasındaki boşluğun doldurulmasında onlara olan gereksinim bir kez daha ortaya çıkmıştır. Bu derlemenin amacı, COVID-19 pandemisinde okul sağlığı hemşirelerinin birincil, ikincil ve üçüncül koruma düzeylerinde rol ve sorumluluklarını bütüncül bir bakışla sunmaktır. Abstract The COVID-19 infection spread around the world in a short time and was declared a pandemic by the World Health Organization in early 2020. To control the epidemic, some decisions affecting all segments of the society have been implemented by governments; such as curfews and travel restrictions, community activity cancellations, working from home, distance education and limiting some health services other than emergencies in hospitals. One of the measures affecting children’s lives and families is the school closures. With the closure of schools, face-toface education of children has been interrupted, and families have faced difficulties in accessing education services due to schools with inadequate technological infrastructure and lacking sufficient technological equipment. Also, there have been problems in accessing school health services such as nutrition, vaccination, and maintenance of health monitoring and controls provided to children and families. In this process, it has been reported that domestic problems, especially violence cases, have increased with long-term home quarantines. Depending on the course of the pandemic, schools have been reopened partially or full-time in some countries. Certain regulations have been made for the reopening of schools in our country and a series of individual and institutional measures and practices have become a current issue for pandemic control. Since there are no school health nurses in our country, especially in public schools, teachers have undertaken the control of the pandemic in many schools. Nursing services, which have increased in importance all over the world during the pandemic, are also essential for the school community. School health nurses, who are professional health workers, have a key role in protecting and sustaining the health of the school community, which constitutes a significant portion of the country’s population. Uluslararası Hemşireler Yılında COVID-19 Pandemisi: Türk Hemşireler Derneği Çalışmaları

Dünya Sağlık Örgütü tarafından pandemi olarak ilan edilen Covid-19 salgınında, tüm dünyada olduğu gibi ülkemizde de, en büyük görev sağlık çalışanlarına düşmüştür. Sağlık çalışanları içinde en ön safta Covid-19 mücadelesine katılan hemşireler, sağlık sistemini işlevsel tutan yapı taşı olmuştur. Hemşirelik mesleğinin toplum yararına üst düzeyde nitelikli ve güvenilir hizmet verecek özerk bir sağlık mesleği olması için güncel ve bilimsel veriler doğrultusunda çalışmalarını sürdürmekte olan Türk Hemşireler Derneği, pandemi ile mücadelede aktif bir şekilde yer almış, hem ulusal hem de uluslararası düzeyde, pek çok kapsamlı faaliyette bu . . .lunmuştur. Türk Hemşireler Derneği’nin faaliyetleri toplum ve sağlık çalışanı güvenliğini sağlama hedefiyle, şube ve il temsilcileri aracılığı ile gerçekleştirilmiştir. Pandemi sürecinin yönetiminde dinamik bir yaklaşım sergileyen Türk Hemşireler Derneği bu dönemde, topluma, meslektaşlarına, karar vericilere ve medyaya yönelik ulusal düzeyde, bilgi ve veri paylaşımında bulunmuştur. Türk Hemşireler Derneği aynı zamanda faaliyetlerini büyük bir katılım ve iş birliği ile sürdürmek için, meslektaşlarının yanı sıra, diğer hemşirelikte özel dal dernekleri ve diğer sağlık meslek örgütleri ile de çalışmıştır. Bu doğrultuda yürütülen tüm faaliyetlerde, sağlık çalışanlarının güvenliğinin sağlanması, toplumun ve sağlık çalışanlarının bilimsel ve doğru bilgiye ulaşması, ülkemizin her yerinde hizmet veren hemşirelere görevlerini yürütürken rehberlik etme ve hastalar için daha güvenli ve nitelikli bir bakım hedeflenmiştir. Bu makalede uluslararası hemşireler yılı gibi çok özel bir yılda yaşanan Covid-19 pandemisinde, hemşireliğin ve Türk Hemşireler Derneği’nin yürüttüğü faaliyetler konusunda bilgi verilmiştir. Nurses who are at the forefront in the fight against COVID-19 have become the building blocks keeping the health system functional. The Turkish Nurses Association, which continues to work in line with current and scientific data in order to make the nursing profession an autonomous health profession that will provide high quality and reliable service for the benefit of the society, has been actively involved in the fight against the pandemic both at national and international levels. The Turkish Nurses Association’s activities have been carried out by means of local representatives, with the aim of ensuring the safety of society and health workers. Demonstrating a dynamic approach in the management of the pandemic process, The Turkish Nurses Association shared information and data at the national level with the society, colleagues, decisionmakers, and the media. The Turkish Nurses Association has also been working with other specialized nursing associations and other healthcare professional organizations, in order to continue its activities with great participation and cooperation. In all activities carried out in this direction, it is aimed to ensure the safety of healthcare workers, to provide scientific and accurate information to the society and healthcare professionals, to guide nurses serving all over our country while performing their duties, and to administer safer and more qualified care for patients. Ortaya çıkan küresel pandemiler bireyler ve topluluklar için yüksek riskler oluşturmaktadır. COVID-19 pandemi sürecinde, hastalığın yayılma hızını yavaşlatmak, hastanelerin artan hasta yükünü düşürmek ve yoğun bakım gereksinimini azaltmak amacıyla fiziksel mesafenin korunması ve sokağa çıkma yasağı gibi günlük yaşama dair kısıtlamalar tüm dünyada uygulanmaya devam etmektedir. Bu önlemler kapsamında üniversiteler yüz yüze eğitim ve araştırma faaliyetlerine ara vermiş ve bu değişiklik bilinen şekliyle yürütülen eğitim ve öğretim faaliyetlerini kesintiye uğratmıştır. Hemşirelik eğitiminde de kısa sürede bu değişikliklere en etkili bir şekilde uyum sağlamak ve uzaktan eğitim yöntemleri ile eğitim faaliyetlerini yürütmek amacıyl . . .a gerekli düzenlemeler yapılmıştır. Hemşire akademisyenler eğitici rollerini bu yeni sisteme göre adapte ederek, hemşirelik müfredatının içeriğini uzaktan eğitim teknolojileri ile sunmak zorunda kalmıştır. Daha önce eşi benzeri hiç görülmemiş bir değişimi beraberinde getiren bu süreç, öğrenciler, eğiticiler ve üniversiteler açısından benzersiz bir deneyim olmaya devam etmektedir. Bu makalede, COVID-19 pandemi döneminde özellikle hemşirelik programındaki teorik dersleri planlama ve yürütmeye ilişkin deneyimler literatür eşliğinde paylaşılmıştır. During the COVID-19 pandemic, in order to decelerate the speed of transmission, to decrease increased patient load of hospitals, and to reduce intensive care need; restrictions have been implemented in daily life all over the world such as curfews and physical distancing. Within the scope of these measures, universities suspended faceto-face education and research activities, and this change interrupted the education and training activities carried out as previously known way. Necessary arrangements have also been performed in nursing education to adapt to these changes effectively in a short time and to carry out educational activities with distance education methods. Nurse academicians had to deliver the nursing curriculum’s content with distance education technologies by adjusting their educational roles. This process, which brings an unprecedented change, continues to be a unique experience for students, educators, and universities. Engelli Çocuğa Sahip Kadınların COVID-19 Sürecindeki Deneyimleri: Nitel Bir Araştırma

ÖZ Amaç: Engelli çocuğa sahip kadınların COVID-19 sürecinde yaşadıkları sorunları ve yaşam deneyimlerinin feminist sosyal hizmet bağlamında fenomenolojik yaklaşım ile ortaya çıkarılması amaçlanmaktadır. Gereç ve Yöntem: Kadınların engelli çocuklarına bakım konusunda COVID-19 pandemisinde yaşadıkları deneyimlerini feminist sosyal hizmet bağlamında derinlemesine incelemek amacıyla araştırmada nitel araştırma yöntemlerinden fenomenolojik yaklaşım kullanıldı. Katılımcılar nitel araştırmalarda kullanılan amaçlı örnekleme kapsamında kartopu yöntemi ile belirlendi. Veriler MAXQDA20 programı ile nitel içerik analizi ile analiz edildi. . . .Bulgular: Araştırmanın sonucunda iki ana tema belirlendi. Bunlar: “COVID-19 sürecinde yaşanan sorunlar ve değişimler”, “COVID-19 sürecinde ihtiyaçlar ve öneriler” şeklindedir. Kadınlar bu süreçte bakım yükü, ev içi sorumluluklar, güce karşı güçsüzlük ve artan gelecek kaygısı yaşamaktadırlar. Ayrıca hem engelli çocuklarına hem de kendi ihtiyaçlarına yönelik talepte bulunmakta ve öneri getirmektedirler. Engelli çocuğa sahip kadınların COVID-19 pandemisindeki deneyimleri incelendiğinde sistematik olarak kadınların çocukların tüm bakım yüklerini üstlendiği, bu nedenle COVID-19 sürecinde oldukça yıprandıkları ve zorlandıkları görüldü. COVID-19 pandemisi başladıktan sonra kadınların kendilerine vakit ayıramadıkları, sosyal çevrelerinden tamamen uzaklaştıkları ve destek sistemlerine ulaşamadıkları ortaya çıktı. Sonuç: Konuya ilişkin yeni çalışmaların yapılması ve kadınların desteklenmesi gerekmektedir. Dolayısıyla özellikle Türkiye gibi gelişmekte olan ülkelerde insan hakları ve sosyal adalet ilkelerinin gözetilerek bu konunun daha fazla irdelenerek, kadının güçlenmesine, destek sistemlerinin arttırılmasına ve bunun için çeşitli düzeyde sosyal hizmet uygulamasına ihtiyaç bulunmaktadır. ABSTRACT Objective: It is aimed to reveal the problems and life experiences of women with disabled children during the COVID-19 pandemic with a phenomenological approach in the context of feminist social work. Material and Method: Qualitative research method was used in the research in order to examine women’s experiences in caring for their disabled children during the COVID-19 pandemic in the context of feminist social work. In this context phenomenological approach was used in the context of qualitative research method. Participants were determined by the snowball method within the scope of purposive sampling used in qualitative research. The data were analyzed by content analysis with the MAXQDA20 program. Results: As a result of the research, two main themes were determined. These are: "Problems and changes in the COVID-19 process", "Needs and suggestions in the COVID-19 process". In this process, women experience the burden of care, domestic responsibilities, powerlessness to power and increasing anxiety about the future. In addition, they make requests and offer suggestions for both their children with disabilities and their own needs. The experiences of women with disabled children during the COVID-19 pandemic were examined, it was seen that women systematically assumed all the care burdens of children, therefore they were quite burn out and had difficulties in the COVID-19 pandemic. It turned out that after start of the COVID-19 pandemic, women could not spare time for themselves, completely distances themselves from their Social environment and could not reach support systems. Conclusion: It is necessary to carry out new studies on the subject and to support women.
